*Taxpayers obtaining health insurance through the "Marketplace" must provide form 1095-A. See the link on our website for instructions on obtaining this tax form.
If Medical Insurance is paid through an employer or you have insurance through a retirement plan or social security no documentation is required.
For those who private pay for their health insurance, provide the total costs of health insurance and proof of health insurance as reported on form 1095 which is a tax form issued by your health insurance provider.
